The metric hair transplant clinics ignore: show-rate
Booked consultations mean nothing if half do not show. What actually moves show-rate from 50% to 80% for hair transplant clinics.

Every clinic dashboard tracks cost per lead. Most track cost per booked consultation. Almost none track cost per attended consultation, which is the only number that pays wages.
Show-rate is the highest-leverage lever in the whole funnel, and it is the one most clinics do not touch because the fix lives in operations, not media.
The maths nobody writes down
Two accounts, same £4,500 monthly spend, same £45 booked-consult cost. One hundred bookings each.
- Clinic A runs at a 50% show-rate. 50 attended. Real cost per attended consult: £90.
- Clinic B runs at an 80% show-rate. 80 attended. Real cost: £56.
Same ad account. Same creatives. £34 gap per attended consult, before you get to close rate. On a clinic doing 100 bookings a month, that is £40,800 a year of media budget that either becomes patients or becomes air.
Deposits do most of the work
A refundable £50 deposit lifts show-rate by 20 to 40 points in almost every account we have taken over. A non-refundable £100 deposit lifts it further, at the cost of a 10 to 15% drop in booked volume.
For any clinic with an average ticket above £2,500, the trade is net positive every time. Fewer bookings, more attendance, cleaner calendar, better conversations.
The only clinics that should skip deposits are the ones running a pure volume play at sub-£1,500 tickets. Everyone else is paying to keep their calendar full of ghosts.
Reminders that actually move the number
The reminder stack that works, in this order:
- WhatsApp confirmation the moment the slot is booked, from a human, not an automation that says "confirmed".
- A 30 to 60 second voice note from the surgeon or lead consultant, 24 hours before. This one thing lifts show-rate more than any SMS chain.
- An SMS reminder two hours before, with the WhatsApp thread linked.
- A rebooking link in the same thread, one tap, no login, no calendar picker with 30 empty slots.
Email reminders do nothing. Calendar invites do nothing. The channel is WhatsApp because that is where the patient already lives.
The 15-minute no-show window
When someone does not show, the recovery window is 15 minutes, not 24 hours. Message on WhatsApp, offer two specific slots in the next 48 hours, hold the deposit against the new slot.
Roughly 30 to 40% of no-shows will rebook if you catch them inside that window. After 24 hours it falls under 10%.
Most clinics send a "sorry we missed you" email the next morning. That is the sound of money leaving.
What we ignore
- Confirmation phone calls that leave voicemail. Adds work, moves nothing.
- Automated reminder emails with a "confirm your attendance" button. Nobody clicks them, and the ones who do would have shown anyway.
- Rescheduling flows that require an account. Any friction on the rebook path is a leak.
- Free consults with no deposit at premium ticket sizes. You are training patients to treat the slot as disposable.
The number that matters
Cost per attended consultation, split by market, tracked weekly.
If your CRM cannot produce that number in one screen, that is the first fix. Everything downstream, from bid strategy to creative testing, is aiming at the wrong target until it does.
